6 Quick Apply Full-time 1 hour ago Full Job Description Open Door is seeking a full-time Care Coordinator for the Cancer Services Program (CSP).
CSP is a New York State Department of Health program that assists patients who don't have health insurance in obtaining high-quality breast, cervical and colorectal cancer screenings.
This includes covering the cost of the screenings, any follow up care that may be necessary, and helping the patient navigate the health care system, including helping address and find solutions to any barriers to care the patient may have.
Care Coordinators are responsible for working with the patients to see if they are eligible for the program, enrolling them in the program, helping make appointments for the appropriate services with our partner providers, collecting all the necessary paperwork and medical documentation that is required and then entering their screening and follow up services, along with the results of those services, into our database.
The Care Coordinator also consults with the CSP Clinical Coordinator on any clinical issues and facilitates discussions with health care providers in our partner sites.
The ideal candidate will be detail oriented, have great communication skills, be familiar with the health care environment, be a team player and have the desire to help underserved patients get the care they deserve.
Responsibilities Works with Cancer Services Program participating providers and sites and other community resources to become knowledgeable about available services that may be helpful to patients Assists in overcoming logistical barriers patients may have in attending their scheduled appointments Provides education to patients to help them understand the importance of cancer screenings, follow ups and treatment referrals.
Contacts and reminds patients about appointments for services Collects information required to enroll patients, information about the medical services they receive and any follow up services that might be needed Completes timely and accurate data entry about those services in our database Facilitates access to available resources for obtaining insurance coverage, charity care and/or sliding fee benefits for patients who are not eligible for CSP Helps ensure that appropriate information is available in the patient's medical record during scheduled appointments Coordinates with patient and health care providers to facilitate timely follow-up services Coach patients to become advocates for their own care Builds professional relationships with community contacts, patient advocates and providers to educate them about CSP and how to identify patients who are at risk with barriers to care Communicates with providers about unique patient needs (i.

, cultural needs) Performs other duties as assigned Qualifications Bachelor's Degree in health care or human service-related field or a minimum of two years of experience preferred Four or more years' experience in cancer screening or health-related screening programs strongly preferred Experience working in medical settings and interacting collaboratively with medical teams required Knowledge about healthcare systems and healthcare terms is required Knowledge about the breadth and accessibility of community resources is required Working knowledge of HIPAA regulations is required Must have a valid driver's license and be able to travel throughout the service area There are many fantastic benefits to a career at Open Door Family Medical Centers.
